INTRODUÇÃO:

Hipomineralização molar-incisivo (HMI) e hipomineralização de segundos molares decíduos (HMD) são defeitos de desenvolvimento do esmalte dentário que apresentam etiologia multifatorial, cuja influência genética permanece controversa. OBJETIVO: Determinar a associação entre fatores genéticos e HMI ou HMD por meio de revisão sistemática de literatura. METODOLOGIA: A pesquisa bibliográfica foi realizada de acordo com o PRISMA Statement em quatro bases de dados: MEDLINE-PubMed, Scopus, EMBASE e Web of Science, além de buscas manuais e na literatura cinzenta. O software Endnote Web® foi utilizado para remoção de artigos duplicados e gerenciamento de referências. A seleção dos artigos ocorreu pela leitura de títulos e resumos e, posteriormente, texto na íntegra. Duas pesquisadoras selecionaram os artigos de forma independente. Foram incluídos estudos observacionais que avaliaram associação entre HMI ou HMD e padrão de herança familiar e/ou variantes genéticas. Foram excluídos estudos: sem grupo de comparação; que não distinguiram HMI e HMD de outros defeitos de desenvolvimento do esmalte; e cujos dados estavam incompletos ou duplicados. Um terceiro examinador foi envolvido quando houve discordância. O risco de viés dos estudos foi avaliado utilizando a JBI Critical Appraisal Checklist para estudos transversais, e a escala de avaliação de qualidade Newcastle-Ottawa, para casos-controle e coortes. A certeza da evidência foi avaliada usando a classificação de recomendações Critérios de Grading of Recommendations Assessment, Development and Evaluation (GRADE). A extração de dados foi realizada utilizando planilha Excel®, e análise independente foi desenvolvida para cada desfecho avaliado. Não foi possível executar metanálise. RESULTADOS: Foram incluídos 14 estudos, sendo que 11 tiveram como desfecho HMI, dois HMD e um artigo incluiu os dois desfechos. Dos 12 artigos cujo desfecho foi HMI, nove avaliaram genes/ variantes genéticas/ interações entre genes, dois avaliaram concordância entre gêmeos mono (MZ) e dizigóticos (DZ), e um fez análise de segregação. Foi identificada associação entre HMI e genes/ variantes genéticas relacionados a amelogênese (AMBN, MMP20, TUFT1, TFIP11, AMELX, ENAM, BMP2, BMP4, BMP7, DLX3, FAM83H, FGFR1 e SCUBE1/TTL12), resposta imune (DEFB1, TGFBR1), outros genes (PPARGC1A/ MIR573, RMI1/ SLC28A3, KCNG1/ NFATC2, CDH5), além de interações entre genes da amelogênese e da resposta imune (TGFA e IRF6, AMELX e IL4/ IL17A/ IL10A/ IL1A/ STAT1, TUFT1 e IL4, BMP2 e IL4) e SNPs no gene de aquaporinas (AQP5) e receptores de vitamina D. Foi encontrada maior concordância de HMI nos pares de gêmeos mono do que nos dizigóticos, e a herdabilidade de HMI foi de 20%. Entretanto, não foi observada associação entre genética e HMI no estudo que analisou a segregação. Três estudos selecionados apresentaram HMD como desfecho e foi observada associação entre HMD e SNPs no gene HIF-1, relacionado à hipóxia. Porém, não foi encontrada diferença entre a concordância de ocorrência de HMD entre os gêmeos MZ e DZ. CONCLUSÃO: Os resultados da presente revisão sistemática demonstraram associação entre genes/variantes genéticas e interações entre genes da amelogênese, resposta imune, além de genes envolvidos no transporte de nucleotídeos, processos metabólicos e interação célula-célula e HMI e HMD.

 

ABSTRACT: INTRODUCTION: Molar-incisor hypomineralization (MIH) and hypomineralized second primary molar (HSPM) are developmental defects of dental enamel that have a multifactorial etiology, whose genetic influence remains controversial. AIM: To determine the association between genetic factors and MIH or HSPM through a systematic review. METHODOLOGY: The literature search was performed according to the PRISMA Statement in four databases: MEDLINE-PubMed, Scopus, EMBASE and Web of Science, in addition to manual and gray literature searches. Endnote Web® software was used to remove duplicate articles and to manage references. The selection of articles was performed first after the reading of the titles and abstracts and, later, the full text. Two researchers independently selected the articles. Observational studies that evaluated the association between MIH or HSPM and family inheritance pattern and/or genetic variants were included. Studies without comparison group; which did not distinguish MIH and HSPM from other enamel developmental defects; and those whose data were incomplete or duplicated were excluded. A third examiner was involved when there was disagreement. The risk of bias of the studies was assessed using the JBI Critical Appraisal Checklist for cross-sectional studies and the Newcastle-Ottawa quality assessment scale for case-control and cohort studies. The certainty of the evidence was assessed by using the Grading Criteria of Recommendations Assessment, Development and Evaluation (GRADE) recommendation rating. Data extraction was performed using an Excel® spreadsheet, and independent analysis was performed for each outcome assessed. It was not possible to perform a meta-analysis, as there was no inclusion of at least two studies using the same exposure. RESULTS: Fourteen studies were included. 11 had MIH as the outcome, two had HSPM and one article included both outcomes. Of the 12 studies in which MIH was the outcome, nine evaluated genes/genetic variants/gene interactions, two evaluated concordance between mono and dizygotic twins, and one performed segregation analysis. An association was identified between MIH and genes/genetic variants related to amelogenesis (AMBN, MMP20, TUFT1, TFIP11, AMELX, ENAM, BMP2, BMP4, BMP7, DLX3, FAM83H, FGFR1 and SCUBE1/TTL12), to immune response (DEFB1, TGFBR1) and other genes (PPARGC1A/MIR573, RMI1/SLC28A3, KCNG1/NFATC2, CDH5). Besides, it was observed interactions between MIH and amelogenesis and immune response genes (TGFA and IRF6, AMELX and IL4/ IL17A/ IL10A/ IL1A/ STAT1, TUFT1 and IL4, BMP2 and IL4) and SNPs in the aquaporin gene (AQP5) and vitamin D receptors. Higher agreement of MIH was found in monozygotic than in dizygotic twin pairs, and the heritability of MIH was 20%. However, no association was observed between genetics and MIH in the study that analyzed segregation. Three selected studies presented HSPM as an outcome and an association was observed between HSPM and SNPs in the HIF-1 gene, related to hypoxia. However, in the analysis of agreement between twins, no difference was found between the occurrence of HSPM between monozygotic and dizygotic twins. CONCLUSION: The results of the present systematic review showed an association between genes/genetic variants and interactions between amelogenesis genes, immune response, as well as genes involved in nucleotide transport, metabolic processes and cell-cell interaction and HMI and HMD.
