Introdução: As inúmeras condições impostas pela COVID-19 impactaram o padrão epidemiológico das fraturas faciais. Em vista disso, o uso e a pesquisa epidemiológica para o planejamento estratégico da saúde são altamente recomendados pela Organização Mundial da Saúde. Objetivo: Este estudo de coorte retrospectivo teve como objetivo identificar as repercussões do distanciamento social durante a pandemia da COVID-19 sobre a epidemiologia das fraturas faciais ocorridas em uma capital do Brasil. Método: Esta pesquisa consiste em um estudo de coorte retrospectivo referente ao ano de 2019 e 2020. A amostra foi composta de 406 prontuários médicos (n=279 — coorte 2019; n=127 — coorte 2020). A variável primária da pesquisa foi o ano de diagnóstico da fratura. As variáveis desfechos foram tipo de fratura, etiologia e gravidade da lesão (FISS — Escala de severidade do trauma facial). Foi realizada uma análise descritiva dos dados, com posterior aplicação de testes bivariáveis entre as variáveis categóricas com nível de significância estabelecido em p<0,05. A análise de variância de um fator em postos foi usada para comparar as categorias das variáveis (mais de 2) em relação à variável numérica (pontuação FISS) em p<0,05 (IC 95%). Resultados: O sexo masculino foi mais afetado. A frequência de fraturas faciais foi menor durante as políticas de distanciamento social (n=127 em 2020) em relação a 2019 (n=279). As fraturas mais frequentes foram do osso zigomático (47,3%-2019; 43,3%-2020). O acidente de moto (39,4%-2019; 42,5%-2020) foi a etiologia mais frequente nos dois períodos. Em relação à gravidade das fraturas, a média da pontuação FISS foi superior em 2019 em comparação a 2020 (2,48 contra 2,44; p<0,05 — IC 95%). Em ambos os anos, as fraturas do osso zigomático apresentaram proporção estatisticamente significativa em relação aos acidentes de moto (p<0,05; IC 95%). Conclusão: A frequência e gravidade das fraturas faciais foram menores durante o período da pandemia da COVID-19, tendo como principal fator etiológico os acidentes de moto.

ABSTRACT

Background: The numerous conditions imposed by COVID-19 impacted the epidemiological pattern of facial fractures. In view of this, the use and epidemiological research for strategic health planning are highly recommended by the World Health Organization. Objective: This retrospective cohort study aimed to identify the repercussions of social distancing during the COVID-19 pandemic on the epidemiology of facial fractures that occurred in a capital city of Brazil. Method: This research consists of a retrospective cohort study for the years 2019 and 2020. The sample consisted of 406 medical records (n=279 — 2019 cohort; n=127 — 2020 cohort). The primary research variable was the year of fracture diagnosis. The outcome variables were type of fracture, etiology and severity of the injury (FISS — Facial Trauma Severity Scale). A descriptive analysis of the data was performed, with subsequent application of bivariate tests between categorical variables with a significance level set at p<0.05. One-way analysis of variance at ranks was used to compare the categories of variables (more than 2) against the numerical variable (FISS score) at p<0.05 (95% CI). Results: Males were more affected. The frequency of facial fractures was lower during social distancing policies (n=127 in 2020) compared to 2019 (n=279). The most frequent fractures were of the zygomatic bone (47.3%-2019; 43.3%-2020). Motorcycle accidents (39.4%-2019; 42.5%-2020) was the most frequent etiology in both periods. Regarding fracture severity, the average FISS score was higher in 2019 compared to 2020 (2.48 versus 2.44; p<0.05 — 95% CI). In both years, zygomatic bone fractures showed a statistically significant proportion in relation to motorcycle accidents (p<0.05; 95% CI). Conclusion: The frequency and severity of facial fractures were lower during the COVID-19 pandemic period, with motorcycle accidents as the main etiological factor.
